NAME OF APPLICANT: St. Tammany Parish Government Attn: Christopher Tissue, 620 N. Tyler St. Covington, LA 70433.

LOCATION OF WORK: The site is located 74422 Highway 1077 Covington, LA 70435 in Sections 21, Township 6 South, Range 10 East, St. Tammany Parish, Louisiana (Latitude: 30.515681, Longitude: -90.203578), as shown within the attached drawings. (Hydrologic Unit Code 08090201, Liberty Bayou-Tchefuncta Watershed).

CHARACTER OF WORK: The proposed project is to expand an existing wastewater treatment plant to provide additional capacity for central sewer service to surrounding residences and new approved subdivision developments.
