NAME OF APPLICANTGARY GARBER, c/o Delta Coast Consultants, LLC, Attn: Alan Jolly, 4924 Louisiana Highway 311, Houma, Louisiana 70364.

LOCATION OF WORKBayou Grand Caillou, located at 8177 Shrimpers Row, Dulac, Louisiana, (29.373677, -90.717805), in Terrebonne Parish, as shown on the enclosed drawings, within Hydrologic Unit Code 08090302, West Central Louisiana Coastal, Louisiana.

CHARACTER OF WORK The applicant has requested Department of the Army authorization to install and maintain a 68-ft long by 90-ft wide recreational dock with boat slip, with a recreational boil room and elevated bathroom in Bayou Grand Caillou.  The structure extends 65’ 4” channelward where Bayou Grand Caillou’s width is approximately 325 feet wide.  The original permit, issued on March 21, 2011, authorized the applicant to install and maintain a dock, rip-rap and appurtenant facilities. The applicant constructed an additional walkway to create a 12-ft wide boat slip, an elevated bathroom with sewer connections and additional decking channelward.  The permitted rip-rap for erosion control was not required, and therefore, not installed.  The applicant constructed the structure and was found to be non-compliant with the permit conditions.  No dredging or fill was required and no special aquatic sites were affected by this project.
